 L2 visa processing

 Could you please someone let me know the process of applying L2(spouse of L1) visa from Nepal.
Should I need to apply from USCIS or directly from kathmandu via online DS-160 form.
or anything more, I need to do from US, please advise if any one aware of the situation.
