 US Embassy kept the passport after saying the visa is approved

My parents had applied for US visa to attend my graduation this August. They were said that visa had been granted and were told to collect their passports today. However, only my mum got her visa today and they kept my my dad's passport. They told him that he'd get a phone call and then he can receive the passport. 

Any idea what is all this mess about ?

Maybe they kept his visa for hold period. But, then again they were told they got their visa means No worries. He will get his passport soon.
On a separate note: They lost your fathers passport and they are still searching for it? lol
