Community Center Health Support Worker Reports to: Resource Center Manager Status: full time, regular, non-exempt or on-call, temporary, non-exempt Salary: $12.00 hourly, depending upon experience (additional bilingual English/Spanish differential offered) GENERAL SUMMARY The Community Center Health Support Worker (CCHSW) is part of an interdisciplinary team who are committed to improving the quality of life for Tenderloin residents. Under the supervision of the resource center manager and the assistant resource center manager, the CCHSW is responsible for implementing the community outreach/drop- in center program. The community center serves as a low-threshold engagement for the Tenderloin’s homeless and marginally-housed residents for whom without such support they may not access many of the health, housing and social supports to which they qualify and are in place to address their need(s). Since the community center is a central program in the agency, the CCHSW works closely on projects across all services areas, including health promotion, social services, healthcare, behavioral health, housing, as well as volunteer and employment training within the community resource center.
The CCHSW operates the community space, including client registration, information and referral, morning snacks, shelter information and reservations, housing information, health information, facility cleanliness and safety, peer support groups, supervision of assigned volunteers, as well as information about neighborhood and city services, and opportunities. An ability to work with and deal effectively with members of the community being served is required. As a daily operation, candidates must be willing to work some evenings and weekends.
ESSENTIAL FUNCTIONS
Client services
· Opens agency and sets up for morning drop-in services
· Closes agency including service area break down and set up for next morning
· Ensures security of facility at closing including setting security systems
· Implementation of community center programming, peer support, hygiene, and social health initiatives
· Oversees all aspects of the community center as assigned in the absence of the Community Center Manager and/or Assistant Community Center Manager
Information and referral
· Conducts reception, initial screening of client needs and registers clients for services
· Provides information and referral services to clients
· Conducts outreach activities publicizing the agency services
· Represents program at community meetings, as assigned
Facility upkeep
· Maintains safe, sanitary, orderly, and clean facility
Documentation
· Maintains documentation, within prescribed deadlines, for all services delivered according to standards set by the agency and as required by public and private funders.
Facilitation
· Facilitates workshops and peer education/skills building groups
Other duties
· Works positively with volunteers independent of the volunteers’ experience and skill levels
· Supervises, trains and supports assigned community center volunteers
· Other duties as assigned.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
· Experience working with active substance users and those experiencing mental health problems utilizing an understanding of harm reduction principles and practices.
· Must be willing to provide support, education, and advocacy and develop relationships following harm reduction principles with the diverse residents from the Tenderloin neighborhood.
· Skill in working with people of color, demonstrated sensitivity towards people from diverse ethnic, cultural, economic, sexual orientation and gender identification communities, the undocumented, the triple diagnosed, and people with disabilities including HIV/AIDS, required.
· Demonstrated knowledge of basic HIV/AIDS information including basic medical and treatment modalities and HIV/AIDS related issues (risk reduction, STDs, hepatitis C, sexuality).
· Excellent people handling skills for working with clients, volunteers, and staff members comprised a multi-disciplinary team of diverse backgrounds and abilities.
· Demonstrated ability to work well under pressure required.
· Excellent verbal and telephone skills.
· Well-organized and detail-oriented with a problem-solving approach.
· Understanding that client confidentiality is mandated and vital for position.
· Knowledge of general office practices and procedures.
· Ability in operating various office equipment, such as personal computer, calculator, copy machine, facsimile machine, typewriter and telephone system.
· Ability to work independently, take initiative, prioritize duties and work closely with several different departments.
· Ability to offer support and services without judgment towards clients.
· Ability to engage in open and frank discussions regarding an array of sexual and drug using behaviors.
· Ability to maintain confidentiality.
· Ability to communicate with clients, employees, volunteers and other business contacts in a courteous and professional manner.
· Ability to work in diverse team settings.
· Ability to maintain schedules.
· Ability to present oral and written reports.
· Ability to hear and speak well enough to converse over telephone 100% of the time.
· Ability to see well enough to use computer efficiently and read computer reports and correspondence 100% of the time.
· Ability to lift up to 15 lbs. occasionally, (example: food boxes).
· Ability to perform routine bending/stooping while setting up and breakdown facility for clean up.
· Ability to perform routine twisting/reaching while working at computer/desk.
· Ability to perform routine and extensive walking/standing during course of day.
PREFERRED KNOWLEDGE, SKILLS AND ABILITIES
· Bi-lingual (English/Spanish)
· Completion of community job or peer training program and/or an internship in a human services agency (providing client services);
· Personal work experience with and knowledge of issues impacting our residents, including: homelessness, substance use, mental health, communicable and chronic diseases, specifically HIV/AIDS, Hepatitis, TB and other sexually transmitted diseases.
· Experience in the area of homeless, social and/or mental health services strongly desired.
· Knowledge of San Francisco specialized services including homeless, , substance use, mental health and services preferred.
· Participatory group facilitation and leadership skills.
· Bilingual/bicultural skills preferred (fluency in English and Spanish);
· CPR and First Aid certification preferred; and
· Knowledge of and ability in using personal computers, Internet and database software is helpful (Microsoft Word, Excel, Outlook, Access)
SALARY & BENEFITS
$10.50-$12.00 hourly, depending upon experience (additional bilingual English/Spanish differential offered), temporary, on-call.
