 TPS i-765 filing instruction for OPT students

Do we have a thread here about what goes on these confusing fields of this form??
If i am on opt and i already have an EAD what should i check on "i am applying for"?? Permission to accept employment??

Where can i find the information regarding the uscis office where i filed for my previous EAD? What if i have two previous OPT's?

Eligibility category? Is it (a)(12) ??

Do we have to fill form i-765 worksheet?? Its the one with annual income and expenses and assets!!
